[Accessibility-ia2] accessibleWithCaret - caretOffset

Alex, Why does accessibleWithCaret have an [out] caretOffset?  The user
can QI to IAText and use IAText::offset.  I propose not adding that
redundancy (and thus extra code for the app developers).  -Pete
