SQL to add TMEdeclaration field

As recent examples show, it might be useful to have a way to enclose type
members declarations in 'ifdef' conditions.

We propose to similar way as used for constants, for which we can store the
whole parts of their declarations.

The SQL attached will add TMEdeclaration field to the TypeMemberExtras table
which can be used to store type member declaration.
