I reported this in February to LKML against 2.6.32.3 without
much reaction, see http://lkml.org/lkml/2010/2/2/208
It also happened with the latest Debian Sid kernel (based on
2.6.32.16), so reporting it here, too.

Now it didn't happen during system bootup, but after an uptime of
several days.  The message, as received by our log server:

After this, the machine froze.  A couple of minutes before the freeze
some MySQL dumps were done, but on rather small databases, so those
didn't put serious load on the system.  We didn't manage to reproduce
the issue yet, although it's much similar to the previous one reported
to LKML, which happens frequently.  The referenced code is below:

I don't know why the paths 8:160 and 8:240 were failed, the FC fabric
probably remained intact during the crash, but multipathd logged this:

This definitely looks linked to the BUG in some way.

Path 8:240 does not exist now, but the same system can also run on
another blade, which sees the same LUN as 0:0:1:3 from the storage.

I hope you get an idea how to tackle this.  An unstable NFS server
isn't particularly fun. :(

** Kernel log: boot messages should be attached
The error messages generated by udev running on the inactive paths
overflow the kernel log buffer before rsyslogd starts, so they aren't
