1. rename PLT.atca.2 to Block Device Removal and move to serviceability

2. rename CSM.atca to Multiple Host Syncronized Device Hotswap and move
to clustering

I have rethought the references and believe CSM.atca should probably end
up in the clustering section since it is not a ATCA specific
requirement.  We can write an ATCA specific portion which depends on the
specialized ATCA hardware requirements that would depend on CSM.atca.
