[lsb-discuss] New Problem Report Form

We're going to start taking problem reports in Bugzilla now.  I've 
created a special BZ form for this; it's "live" except for the fact that 
the current PR system doesn't redirect to it yet.  (That'll probably 
happen very soon.)

Here's the form:
