[Openais] lcr take 3 - hdb.h integrated into rest of aisexec

I integrated the static inline version of hdb.h into aisexec and found
performance increases 3% - size increases 1%.  Seems like a good
tradeoff to me.  I think this is because of all of the handle lookups
throughout the totem layer.

Mark, could you have a look over the evt changes and make sure they look
ok to you?  The part where I return an error code to the library may
need a little work...

testevt passes this version.
